• We are required to inform you of our policy on advance directives. All procedures done in this facility are elective therefore it is the conscience and policy of this facility to attempt to reverse any condition that presents itself during your time here. We do inquiry of a patient has an advance directive and ask that a copy be provided for the chart. Should you require transfer to another medical facility for further treatment; the advance directive will accompany you. Should you require information on an advance directive, it can be provided by staff. Information is also available in our lobby.

You Have the Right to:
- Considerate, respectful care at all times and under all circumstances with recognition of your personal dignity
- Personal and informed privacy, within the law.
- Be fully informed concerning procedure, treatment and expected outcomes to the degree known before it is performed.
- The opportunity to participate in informed decisions involving your health care.
- Access to treatment regardless of race, sex, color, national origin, religion, handicap, or disability.
- Confidentiality of records and disclosures. Except when required by law, you have the right to approve or refuse release of records.
- Any other use of this information without the express written consent of the patient is prohibited. These records are protected by federal regulation. Federal rules prohibit us from further disclosure unless you have received written consent from the person to whom it pertains or as otherwise permitted by 42 CFR Part 2.
- Know the identity and professional status of individuals providing service.
- Report and comments concerning the quality of services provided to you during the time spent at the facility and to receive fair follow up on your comments without being subjected to discrimination or reprisal.

- Provide accurate, complete information about present conditions, past illnesses, hospitalizations, medications, and other matters related to your health.
- Follow the treatment plan recommended by the primary physician.
- Indicate whether you clearly understand the contemplated course of action of action and what is expected of you.
- Your actions if you refuse treatment, leave the facility against the advice of your physician or anesthesiologist and/or do not follow the physicians' instructions related to your care.
- Assure that the financial obligations of your health care are fulfilled as expeditiously as possible.
